What the clinic decides before the concept design

Medical centre construction in Batumi depends on decisions the clinic makes before the concept sketch: appointments only or an inpatient ward as well, whether there will be X-ray, an operating theatre, medical gases and heavy diagnostic equipment, and on which floors patients will be seen. The clinic sets the requirements for the premises itself, under its licence or permit, and we translate them into the number of storeys, the column grid, the shafts and the power supply.

If an investor is building and there is no tenant yet, we build in a versatile base: a floor with consulting rooms on both sides of a corridor, risers every few rooms and extra storey height for ducts. We usually give such a building a cast-in-place concrete frame: the consulting room partitions can later be moved without touching the columns. The fit-out for a specific clinic is done to its brief, as in a clinic renovation.

Entrances, stairs and routes at building level

The flows within a floor are laid out by the designer, but entrances, stairs and lifts are built in from the first sketch. Patients come in through the main entrance to reception. Staff, linen, consumables and waste go through a service entrance with its own staircase, bypassing the waiting area. We put the room for temporary waste storage by the service exit, with access for the vehicle that collects the waste.

Heavy diagnostic equipment and anything brought in by crane or on a trolley stays on the ground floor, and doctors' consulting rooms go higher up. Clean zones within a floor and finishes that stand up to disinfection are described on the page about clinic design.

Accessibility: entrance, lift, toilets

Since 1 March 2021, Georgia has had the technical regulation “National Accessibility Standards”, approved by Government Resolution No. 732 of 4 December 2020. It covers buildings of all classes, apart from exceptions such as a private house with one or two dwellings, and we design a new medical centre to it from the first sketch. The architect checks the provisions for your building against the current version.

On the plan this means a step-free entrance or a ramp, doors and corridors wide enough for a wheelchair, a toilet with turning space for a wheelchair on every floor where patients are seen, and a lift if patients go above the ground floor. A 630 kg passenger lift with 4–5 stops costs from 90,000 ₾ installed. If patients will need to be moved lying down, the clinic sets the size of the car, and we build the shaft for it from the start: it can't be widened in a finished frame.

Ventilation and storey height

A medical building needs a higher air exchange rate than an office, and the ducts take up space above every corridor. So we work out the storey height from the ceiling void: ducts, cable trays and pipes have to pass each other without lowering the ceiling in the consulting rooms. The fresh air intake is kept away from the exhaust, the car park and the bins.

We build the vertical ventilation shafts with spare cross-section: a separate one for the toilets and the waste room, and another for the unit serving the treatment block or the operating theatre, if there will be one. Supply and extract ventilation with equipment and installation costs from 205 ₾ per m².

X-ray, medical gases and backup power

We choose the rooms for X-ray and heavy equipment before the slabs are calculated: the structural engineer designs the slab for the weight in the equipment data sheet, and we draw the openings and the delivery route on the plan. Shielding for walls, doors and floor slabs is set by a radiation protection specialist's calculation, and the clinic obtains the permit for the unit itself. Barium plaster or lead sheets cost from 300 ₾ per m² of wall; how shielding is installed without gaps is shown in the example of an X-ray room in a dental clinic.

Oxygen and medical vacuum are designed and installed by a specialist supplier to the clinic's brief, and we leave shafts, routes and a room for the supply source in the building. For equipment that must not be switched off suddenly, we provide space for a generator with an automatic changeover switch and separate circuits in the distribution boards.

Plot, class of structure and acceptance into use

Before the plot is bought, we check whether its zone allows medical use and the number of storeys you need. The class of structure is determined by floor area, height, the depth of the underground part and the spans, and the building takes the class of its highest parameter, as described in the section on commercial construction. For a clinic the lift matters too: under the classification rules, a building with a lift belongs at least to Class III, and in a built-up area the surveys then include a survey of the neighbouring buildings.

Acceptance into use, if it is required, and registration of the building in the Public Registry are prepared together with our partner lawyer. It is best to find out before the contract which papers from the builders the clinic will need for its licence.

How much medical centre construction costs in Batumi

A turnkey medical centre or clinic building costs from 2,880 to 3,600 ₾ per m². The price includes the shell, the facade, a lift, ventilation with a higher air exchange rate, washable finishes in the consulting rooms and corridors, and the toilets. Medical equipment and furniture are not included; X-ray shielding is priced as a separate line to calculation, and medical gases to the supplier's design. The shell of a 1–3 storey building without services or finishes starts from 990 ₾ per m², and a fire alarm, installed, from 17 ₾ per m².

A building with X-ray, an operating theatre and a second lift costs more, and an outpatient consulting centre without water in every room costs less. We send a preliminary estimate based on the plot extract and the clinic's brief within 24 hours on business days. The final cost is set on the basis of the design, after a visit to the plot and once the estimate is agreed.

Stages and timelines

  1. Brief and plot

    3–6 weeks

We get the list of rooms and the equipment data sheets from the clinic, and check the zone, access and utility capacity.

  2. Concept with the clinic

    3–5 weeks

We agree the building's volume, entrances, lift and patient floors with the medical director or the licensing consultant, and record comments in writing.

  3. Design and permit

    3–5 months

Structure with floor slabs for diagnostic equipment, ventilation, wiring with backup power, the document package for the class of structure.

  4. Foundations and frame

    4–6 months

The slab and the services beneath it, columns, floor slabs, the lift shaft, a roof with a platform for the plant.

  5. Facade, lift, services

    3–4 months

Windows and facade, lift installation, ducts, water to the basins, cables, X-ray shielding to calculation.

  6. Finishes

    2–3 months

Consulting room partitions, ceilings, washable walls and floors, toilets, the ramp and the canopy at the entrance.

  7. Commissioning and handover

    2–3 weeks

We set the airflow rates, measure the earthing, test the backup supply and hand over the handover file.

Local specifics in Batumi

On low-lying plots near the shore the groundwater is close to the surface, and after a heavy downpour water can get into a basement. So we don't put the switch room, the generator, the medical gas supply or the archive in the basement: they go on the ground floor or above any possible flood level. For the same reason, heavy diagnostic equipment stays on the ground floor slab, not in the lower ground floor.

Patients are often brought by car, and in a downpour getting out at the entrance becomes a problem. We extend the entrance canopy to where the car stops, build the ramp with drainage and a non-slip surface, and make the entrance lobby deep, so that damp air from the street doesn't reach reception.

The structural engineer designs the building for seismic loads, but in a clinic what stands inside is also dangerous in a tremor. For medical gas cylinders, tall cabinets, heavy equipment and suspended units we leave embedded fixings in the walls and slabs, and their positions are set by the structural engineer and the equipment suppliers at the design stage. We photograph the fixings before the finishes go on, so that the clinic doesn't later drill into walls at random.

Materials and brands

We fill the external walls of the frame with clay blocks or pumice blocks: they are heavier than aerated concrete and absorb street noise better. Consulting room partitions are built on Knauf studs with mineral wool and two layers of board. Windows are on REHAU profiles or an Asistal aluminium system.

Ventilation units come with heat recovery and a dehumidification section, because summer supply air in Batumi is very humid. To air-condition several floors we use a Mitsubishi Electric VRF system. Distribution boards, breakers and the backup supply are assembled with ABB or Schneider Electric. The lift is chosen by class: a car that takes a wheelchair, automatic doors, a link to a monitoring centre, and we choose the brand by the servicing available in Batumi.

What makes up the cost

Timeline: a 600–900 m² medical centre on 2–3 storeys with a lift: preparation, design and permit usually 5–7 months, then another 10–14 months of construction including ventilation balancing

  • parking on your own plot or on the street

What raises the cost

  • X-ray rooms and heavy diagnostic equipment with strengthened floor slabs
  • a second lift or a car for moving a patient lying down
  • a generator and backup power for critical equipment
  • many treatment rooms and a basin in every consulting room

What lowers the cost

  • an outpatient consulting centre without water in every room
  • a typical floor with identical consulting rooms on both sides of the corridor

We prepare the exact estimate from measurements and photos. It’s free and comes with no obligation.

Guarantees

Work has a 2-year warranty, concealed pipes and wiring 5 years. The five years apply to the water pipes, drains and cables in walls and floors, if we laid them. Ventilation and air conditioning lines carry the 2-year warranty on work. The lift, generator, ventilation units and medical equipment are covered by the manufacturer's warranty, and their connection to the building's services, which we carry out, by our warranty on work.

Questions and answers

How much does it cost to build a medical centre in Batumi?

A turnkey medical centre or clinic building costs from 2,880 to 3,600 ₾ per m². The price includes the shell, the facade, a lift, ventilation with a higher air exchange rate, washable finishes and toilets. Medical equipment and furniture are not included; X-ray shielding is priced separately, from 300 ₾ per m² of wall, and medical gases to the supplier's design.

A building with X-ray, an operating theatre and a second lift comes closer to the upper end, and an outpatient consulting centre without water in the rooms closer to the lower end. We send a preliminary estimate within 24 hours on business days, and the final cost is set on the basis of the design, after a visit to the plot and once the estimate is agreed.

Can a clinic building be built before the licence is obtained?

Yes. The clinic obtains its licence or permit for medical practice itself, and construction doesn't get in the way of that. But the requirements for the premises need to be known before the design: which rooms, what ventilation, where the basins go, whether there will be X-ray. Otherwise the building gets built and then starts being altered to fit the application.

The clinic confirms the list of rooms and the data sheets for heavy equipment in writing before the working drawings: the structural engineer calculates the slabs from them, and the services engineers mark out the shafts. The most expensive requirement is one that appears after the slabs have been poured, so any change goes through a contract addendum with its own price and timeline.

Does a two-storey clinic need a lift?

If patients are seen on the upper floor, we provide a lift or a platform lift. We design a new clinic building to the “National Accessibility Standards” regulation: a patient in a wheelchair must be able to reach the doctor without help, and the architect checks which provisions apply to your building against the current version. If the upper floor is used only for staff rooms, the architect checks whether the regulation allows the building to do without a lift.

Bear in mind the paperwork too: a building with a lift falls at least into Class III of structures, and it needs more surveys. A 630 kg passenger lift costs from 90,000 ₾ installed.

Where should X-ray and MRI go in a clinic building?

Usually on the ground floor, on the ground-bearing slab, with a direct route for bringing the unit in from the entrance. A ground-bearing slab carries a heavy unit easily, and there are no rooms below it that would need shielding through the floor. We don't put X-ray right next to the waiting area or the children's corner, even if the shielding would allow it: it is more reassuring for both patients and staff.

An MRI scanner has its own requirements for shielding, the magnetic field and cooling, and they come from the scanner's supplier. We design the room for it to that layout before the slab is poured.

Can part of a medical centre building be let to a pharmacy?

Yes, and a pharmacy on the ground floor by the entrance is usually convenient for patients. We plan it as a separate unit: its own street entrance, a shopfront, separate meters and distribution board, its own ventilation. The pharmacy can then be let to an outside tenant, and its opening hours don't depend on the clinic's schedule.

The requirements for the pharmacy are set by its licence, just as for the clinic, and the fit-out is done to the tenant's brief. Finding a tenant and the lease agreement can later be handed over to us under property management.
